HRS client Deer Meadows Home Health becomes the first home health provider in the United States to achieve CHAP's Disease Program Certification across four disease states

PHILADELPHIA, P.A. – Based on Community Health Accreditation Partner's (CHAP) rigorous standards of excellence, Deer Meadows Home Health and Support Services, LLC becomes the first home health provider in the United States to achieve Disease Program Certification across four disease states: wound care, congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), and diabetes.

To achieve the distinction of CHAP certification, Deer Meadows underwent rigorous program evaluation by CHAP, evaluating program structure and function, quality metrics, advanced staff education, and self-management tools for patients.

A key component of Deer Meadows patient education and self-management tools—and thus their CHAP certification—is Deer Meadows advanced remote patient monitoring (RPM) program.  

In partnership with the nation’s leader in remote patient monitoring, Health Recovery Solutions, Deer Meadows provides daily monitoring of eligible patients—reviewing vital signs and symptoms, prescribing disease-specific education, and managing medication.

The RPM program enables Deer Meadows clinical team to closely monitor patient progress, identify exacerbations, and prevent hospital readmission or ED visits. As well, the program allows Deer Meadows’ clinical team to provide personalized education and coaching to patients, teaching them to manage their condition and care needs in the future.

Patients are evaluated for the RPM program during their first in-home visit and enrolled in the program shortly after, receiving a full education on the RPM program, the devices included in the program, and their personal care plan and goals during the enrollment visit.

“Remote patient monitoring patients enables patients to become more educated and, by participating daily with their home health provider, become an active part of their health care team,” says Stanley Rynkiewicz (DMHHSS Administrator). “Remote patient monitoring aims at improving patient care through digitally transmitted health-related data. That allows early detection of disease decompensation and intervention, patient education and improves patient’s relationship with their health care providers. As part of an RPM program, clinicians can use smart phones or tablets to send patients education modules specific to their needs, along with daily tips on types of foods to eat, medications and why it is important to exercise.”

Remote patient monitoring patients are typically monitored for 30 days and receive follow-up calls post discharge to answer questions that arise and further support them on their journey to disease self-management. Additional support is provided to patients through a pharmacy partnership program developed by Deer Meadows. The program provides three critical services to Deer Meadows patients—supplying medication at an affordable cost, ensuring medication is received by patients, and conducting medication reconciliation to confirm patients are taking the correct medication and dosage. 

In addition to providing patient education, Deer Meadows requires staff to undergo a rigorous training program across the four disease states. The training program provides disease process overview, wound care education, and addition CEU courses.
