("Company", "we", "us", or "our") in connection with remote patient monitoring (RPM) services that we provide on behalf of participating healthcare providers. Please read these SMS Terms & Conditions carefully.
Program Name: Health Recovery Solutions Patient SMS Adherence Program
Health Recovery Solutions, Inc. provides remote patient monitoring support services on behalf of participating healthcare providers. In connection with your provider's RPM program, you may receive recurring automated and non-automated text messages from Health Recovery Solutions, Inc. intended to encourage program adherence and to remind you to take and submit required biometric measurements when they have not been received. Messages may also include technical support responses, program updates, service-related notices, and general adherence guidance.
By enrolling in, participating in, or otherwise agreeing to receive communications in connection with your healthcare provider's RPM program, you agree to receive SMS text messages related to that program from Health Recovery Solutions, Inc. on behalf of your provider. Your mobile telephone number may be provided to us by your healthcare provider, its staff, or through RPM enrollment, registration, or medical record information associated with your care. Consent to receive these text messages is not a condition of purchasing any goods or services.
You represent that you are the subscriber, customary user, or authorized user of the mobile telephone number to which messages are sent. If your mobile telephone number changes, you are responsible for notifying your provider and/or Health Recovery Solutions, Inc. promptly at support@healthrecoverysolutions.com or 551-203-0011 so that your records can be updated and messages are not sent to an unintended recipient.
Message frequency varies based on your individual program requirements and your adherence to taking biometric measurements. Message and data rates may apply. Depending on your text plan, you may be charged by your carrier. Please contact your mobile service provider for pricing and details regarding your specific text messaging plan.

Your privacy is important to us. In connection with the RPM messaging program, we may receive your mobile number and related program information from your healthcare provider or its authorized representatives in order to send service-related text messages on your provider's behalf. We will not share, sell, or trade your text messaging opt-in data and consent with any third parties for promotional purposes. Limitation of Liability

This includes, but is not limited to, damages resulting from delayed, missed, or misdirected messages, or any reliance on the information provided via text message.
We reserve the right to modify these SMS Terms & Conditions or the messaging program at any time. If we make material changes, we will notify you by sending a text message with the updated terms or a link to the updated terms. Your continued receipt of text messages from us following such notice constitutes your acceptance of the modified terms.
These SMS Terms & Conditions and your use of the SMS program are governed by the laws of the State of New Jersey, without regard to its conflict of law principles. Any disputes arising out of these terms shall be resolved exclusively in the state or federal courts located in New Jersey.
