RHTP Defined Areas of Support​

HRS Indirect Fulfillment of Requirement​

1.     Providing payments to health care providers for the provision of health care items or services, as specified by the Administrator.

  • HRS does not manage direct payment to healthcare providers​
  • HRS indirectly supports the requirement of providing payments via enabling billable services under CMS Physician Fee Schedule, such as via RPM, CCM, PCM, APCM, BHI, TCM​
  • Improving provider efficiency by allowing providers to operate top of license and maximize panel capacity​

2.     Recruiting and retaining clinical workforce talent to rural areas, with commitments to serve rural communities for a minimum of 5 years.

  • While HRS is not a direct provider, HRS ourselves enable the retention of clinical workforce talent ​
  • Burnout reduction enabled via RPM to reduce windshield time without reducing quality of care​
  • Digital tools expand clinical reach and impact, improving satisfaction and retention – reduction in fatigue, making rural practice more attractive​

3.     Providing technical assistance, software, and hardware for significant information technology advances designed to improve efficiency, enhance cybersecurity capability development, and improve patient health outcomes.

  • While HRS does not replace or manage a state’s IT backbone, it complements and strengthens IT modernization efforts with turnkey tools and compliance frameworks that align with CMS’ goals​
  • HRS’ Remote Patient Monitoring infrastructure promotes more efficient care delivery aimed in a patient centric manor, aimed at optimizing outcomes for the most vulnerable populations​

4.     Assisting rural communities to right size their health care delivery systems by identifying needed preventative, ambulatory, pre-hospital, emergency, acute inpatient care, outpatient care, and post-acute care service lines.

  • HRS supports a virtual extension of services via RPM, supporting an expansion of clinic and hospital capacity through our solutions, reducing pressure on inpatient beds and enabling flexible care delivery​
  • By preventing unnecessary ED visits and readmissions, HRS indirectly supports the optimization of acute and emergency service lines​
  • HRS programs help rural facilities stabilize margins and focus resources on the right mix of services, complementing state-led right-sizing strategies.​

5.     Supporting access to opioid use disorder treatment services (as defined in section 1861(jjj)(1)), other substance use disorder treatment services, and mental health services.

  • While HRS does not provide treatment of OUD or SUD, HRS’ platform supports virtual visits and remove engagement, helping providers extend access to counseling, therapy, and medication management for OUD/SUD​
  • Promotes health equity and access to care, with cellular enabled devices to enable care in rural populations with limited in person mental health resources​
